- W2610273114 abstract "Background - Gamma scintigraphy was employed toassess thedeposition ofaerosols emitted fromapressurised metered doseinhaler (MDI)contained inamicroprocessor controlled device(SmartMist), a systemwhichanalyses an inspiratory flowprofile andautomatically actuates the MDI whenpredefined conditions offlow rateand cumulative inspired volume coincide. Methods- Micronised salbutamol particles contained inacommercial MDI (Ventolin) werelabelled with99m-technetium usinga methodvalidated bythedetermination of(1)aerosol size characteristics ofthedrugandradiotracer following actuation intoaneight stagecascade impactor and(2)shotpotencies ofthese non-volatile componentsasa function ofactuation number.Usingninehealthy volunteers in arandomised factorial interaction design theeffect ofinspiratory flowrate(slow, 301min;medium,90Vmin;fast, 270/min) combinedwithcumulative inspired volume (early, 300ml;late, 3000ml)wasdetermined ontotal andregional aerosol lung deposition usingthetechnique ofgamma scintigraphy. - The SmartMistfiring atthe mediumlearly setting (mediumflowand early inthecumulative inspired volume) resulted inthehighest lungdeposition at 18-6(1.42)%. Theslow/early setting gave thesecondhighest deposition at 14a1 (2.06)% withthefastllate setting resulting inthelowest (7.6 (1-15)%). Peripheral lung deposition obtained forthemediumlearly (9.1 (0.9)%) andslowlearly (7.5 (1.06)%) settings wereequivalent buthigherthan thoseobtained withtheothertreatments. Thisreflected thelowertotallungdeposition at theseothersettings as no difference in regional deposition, expressed as a volumecorrected central zone:peripheral zoneratio, wasapparent forallmodesofinhalation studied. Conclusions - TheSmartMist device allowedreproducible actuation ofanMDI at a preprogrammed pointduringinspiration. Theextent ofaerosol deposition inthelungisaffected byachangeinfiring point andispromoted byaninhaled flow rateofup to901/min - thatis,theslow andmediumsetting usedinthesestudies.
