• W2613741593 abstract "To improve client-provider communication during family planning consultations the audiotaped transcripts of 178 family planning counseling sessions at 17 service sites throughout Kenya in 1993 were reviewed. Of particular interest were the issues of client participation in sessions decision making and informed choice and counseling of continuing clients. 58 of the 61 service providers were women with an average of 7 years of experience. Transcript review indicated that providers dominated most sessions (72%) with clients tending to speak only in response to questions. Providers rarely asked clients about their reproductive intentions failed to relate available contraceptive methods to the clients personal situation and did not ensure the client understood method use instructions. The option of method switching was offered to only half of dissatisfied continuing acceptors. 25% of continuing and 30% of new clients were classified as playing an active role in the session. When clients made an active speech (volunteered extra information in response to questions requested a method or asked questions) providers responded with technical information 42% of the time offered support or constructive suggestions 28% of the time and ignored the client 22% of the time. Client participation was greatest when providers expressed emotion or related technical information geared to the clients personal needs. Recommendations for improving family planning outcomes include promotion of the concept of the client as active decision maker and the provider as concerned advisor group talks and videos to educate clients about why and how to be more proactive in counseling sessions and training programs to teach providers how to encourage client participation." @default.
