• W2621909189 abstract "Einleitung: Der Morbus Crohn (MC) stellt klinisch ein heterogenes Krankheitsbild dar und führt häufig zu Komplikationen, die eine chirurgische Intervention notwendig machen. Es existieren verschiedene Serummarker für die Differentialdiagnose und Stratifizierung des MC aber deren nicht ausreichende Genauigkeit limitiert den Gebrauch in der klinischen Praxis. ZIELE: Wir haben eine Gruppe von neuen Serumantikörpern gegen Glykan-Epitope bei Patienten mit MC getestet, inklusive der bisher noch nicht publizierten Anti-Laminarin (Anti-L) und Anti-Chitin (Anti-C) Antikörper. Methoden: Serumproben von 809 Patienten des „Kompetenznetzes CED“ (363 CD, 124 Colitis ulcerosa (CU), 74 andere gastrointestinale Erkrankungen und 249 gesunde Kontrollen) wurden auf das Vorhandensein von Anti-L, Anti-C, Anti-Chitobioside IgA (ACCA), Anti-Laminaribioside IgG (ALCA), Anti-Mannobioside IgG (AMCA) und Anti-Saccaromyces cervisiae IgG (gASCA) Carbohydate Antikörpern mittels ELISA untersucht (Glycominds Ltd., Lod, Israel). Klinische Daten über Diagnose, Krankheitslokalisation, Komplikationen und Operationen waren verfügbar. Ergebnisse: Serumspiegel aller Marker waren signifikant höher bei Patienten mit MC im Vergleich zu CU und Kontrollen (p<0,0001). 75% der MC Patienten waren positiv für wenigstens einen Marker. Bei Patienten mit MC und Komplikationen waren die Level von gASCA, ALCA, ACCA, AMCA, Anti-L und Anti-C signifikant erhöht gegenüber MC Patienten ohne Komplikationen. Alle Antikörper waren ebenso erhöht bei MC Patienten mit der Notwendigkeit für Operationen. Zusätzlich war Positivität für mindestens einen oder mindestens zwei Marker mit einer höheren Wahrscheinlichkeit für ilealen Befall (p=0,0005 und p=0,004), Komplikationen (p<0,0001 und 0,0001) oder Operationen (p=0,006 und 0,0004) assoziiert. Eine Analyse nach Quartilen ergab, dass Patienten mit höheren Spiegeln der getesteten Antikörper eine höhere Wahrscheinlichkeit für ilealen Befall (p=0,009), Komplikationen (p=0,0001) und Operationen (p=0,0001) hatten. Schlussfolgerung: Spezifische Kombinationen und Spiegel von Anti-Glykan Antikörpern erleichtern die Diagnose und Differentialdiagnose von MC und sind mit komplizierten Verläufen und Operationen assoziiert." @default.
