- W2623007967 abstract "The study of acquired sensorineural hearing loss (SNHL) has relied heavily on assessments of hair cell injury and loss and the threshold sensitivity losses that accompany them. This strategy is based on the view that, for most acquired SNHL etiologies, sensory hair cells are the most vulnerable cochlear elements and auditory neurons (ANs) degenerate if, and only long after, loss of their peripheral inner hair cell (IHC) targets. Recent work in our laboratories has suggested that, in aging and after noise, the synaptic connections between IHCs and ANs are actually the most vulnerable elements, and that massive cochlear synaptopathy can be seen before hair cell loss or threshold elevation. In characterizing these outcomes, our investigational algorithms have relied on assessments of hair cell and early-stage neural generators of evoked potentials, coupled with otoacoustic emissions, tests of middle ear and medial olivocochlear reflexes and electrophysiological assays of neural temporal fine structure processing. Responses recorded non-invasively facilitate translation of findings in the animal models to clinical diagnostics. We couple these with assessments of histopathology of hair cells, neurons and the synapses that connect them, which, at present, provide the only truly definitive assay for the synaptopathy.
