- W2665254923 abstract "「患者によりそうサイエンス」というテーマを考えるうえで，それがどのようなサイエンスであるか，そのサイエンスがどこまで確立・成熟されてきているかを考慮することが重要である．我々が患者さんと接する医療の現場においては，すでに以前より長い歴史を持って応用されてきている科学技術の成果があり，その直接的影響がすでに隠れて見えないぐらいに浸透している技術分野もある一方で，新しい技術が開発されたことにより，今まさに大きく変化している応用領域も存在している．アレルギー・免疫分野においても，すでに多くの科学技術が応用され，日常診療に利用されているが，診断領域・病態解析領域のみならず，治療薬開発，患者サービスに至るまでの多くの分野での応用開発が進んでおり，患者さんの日々の診療に，よりそう形での利用が可能となってきている．しかしながら，日々の臨床現場での患者さんのサポートを行うことを実行に移そうとした場合，すでに十分に解明・確立されている領域はまだ少なく，稀少難治疾患のみならず，コモンディジーズにおいても，未だ十分解決されていない問題に取り組む必要に迫られていることに気づく．このような問題に取り組み，問題を解決しようとする場合には，従来の既に確立された科学技術を応用するだけでは十分ではなく，いままでにない最先端科学技術に踏み込んでアプローチする必要に迫られることも多い．例えば，昨今の遺伝子医学の進展に伴い，病態と関連した遺伝子変異などが多く明らかになってきているが，遺伝子解析が進みそのDNAの変異が明らかになっても，その変異の意義が十分に解明できないことも少なくなく，臨床現場での患者さんに役立つ診療を展開することは困難である．我々は，これまでタンパク等における原子レベルでの立体構造解析を行ってきており，サイトカイン制御を目指したインターロイキン活性化メカニズム解析・治療薬探索や，免疫異常症への治療戦略としてのペプチド療法へ向けた構造解析と先端技術の開発などを進めてきている．このような先端技術の臨床への応用という課題においては，高度な技術利用自体における問題点も多くあるが，一方では，その成果をいかに浸透させていくかという面でも多くの課題を抱えているのが現状である．現在，当たり前のように使用されている，サイトカイン測定や遺伝子解析についても，かつては臨床現場とは遠く離れたように見えた最先端科学がつないできた結果であることを再考することが重要である．これまでの研究の一部を紹介し，各医師におけるサイエンスの役割，サイエンスと臨床現場をつなぐ上での役割などについて考える．" @default.
