- W2680114637 abstract "目的：検診マンモグラフィ(MMG)におけるカテゴリー3(C-3)の微小石灰化病変について，ステレオガイド下吸引式乳房組織生検(stereotactic vacuum assisted breast biopsy: ST-VAB)の適応に対する乳腺ダイナミック造影MRI (dynamic contrast enhanced MRI: DCE-MRI)の有用性を検討する。方法：ST-VAB を施行した341症例のうち，検診MMGにおいてC-3の微小石灰化を指摘され，DCE-MRIを施行した42症例(異常なし(nothing particular: N.P.)10例，Mastopathy 27例，DCIS 5例)について後方視的に調査を行った。病理組織診断別に，MMGの石灰化部位に相当するDCE-MRIにおける濃染域の形態評価およびwash-in rate (WRin)，washout rate (WRout)，正常乳腺の造影効果を考慮した早期濃染および後期濃染の指標であるSlopeearly, Slopelateの検討を行った。さらに各パラメータの感度，特異度，陽性反応適中率，陰性反応適中率，正診率を算出した。結果：DCE-MRIにおける濃染域の形態は病理組織診断別に有意差は認められなかった。一方，WRinとSlopeearlyはDCIS群が高値を示し，N.P.群とDCIS群およびMastopathy群とDCIS群の間に有意差が認められた。特にSlopeearlyは感度100%，特異度97.1%，陽性反応適中率83.3%，陰性反応適中率100%，正診率97.4%と高い診断能を示した。また，病変部位が造影されなかった3症例は非癌群であった。結語：ST-VABの適応に迷うC-3症例に対してDCE-MRIを追加し，WRinおよびSlopeearlyの評価を行った結果，高値を示した際は積極的にST-VAB を行うべきである。一方，造影効果がない病変はST-VABの施行を省略し，経過観察が可能である。よってDCE-MRIはC-3症例におけるST-VABの適応か否かを検討する上で有用であることが示唆される。" @default.
