- W2684502571 abstract "約50年の歴史をもつワルファリン単独の抗凝固療法は今，新しい時代を迎えつつある．ここ数年の間に抗トロンビン薬，抗Xa薬に関する大規模臨床試験結果が続々と報告され，新しいパラダイムの上で抗凝固療法が可能になるという期待が高まった．この期待は，主にふたつのメリットに集中している．(1)使用利便性：きめ細かな用量調節が不要，毎回の採血モニタリングが不要，食事制限が不要，そして併用薬の注意が最小限となること，(2)頭蓋内出血の減少：脳卒中予防効果は同等であっても，致命的となりやすい頭蓋内出血が減少すること．この時代に生まれたイノベーションは，確かにこれまでの大きな障壁を打ち破り，新規抗凝固薬がワルファリンに取って代わることができる，という希望的憶測を生んだ．しかし，これらの新規抗凝固薬の発売以降を見るとどうだろう．ダビガトラン，リバーロキサバンともに，発売後半年で5名の出血による死亡が報告された．このように副作用が続々と報告されることは，約60年前のワルファリン発売時と同様である．抗凝固薬は細胞を介さず，分子を操作してイベントを予防するという特殊な薬理作用をもっている．「個人差をどうマネジメントするか」という課題は，すべての新規抗凝固薬が負っている宿命であり，育薬のプロセスの中で克服していかなければならないだろう．" @default.
