Conservative treatment will conquer some current domains of surgery, such as inflammatory bowel diseases or metabolic disorders. Functional disorders could potentially benefit from better pharmacological agents, but possibly surgical electrical stimulation could become a realistic alternative option. In oncological diseases, surgery will certainly maintain its elementary role in treatment. However, earlier stages and palliative treatment will be taken over by interventional gastroenterology and radiology. On the other hand, more advanced stages, which are still inoperable today, will benefit from surgery. Surgery of the future will be stimulated by new therapeutic approaches due to local tissue ablation, intraoperative tissue differentiation, new techniques of viscerosynthesis, and improved specimen retrieval. Smart implants and electrical stimulation devices will offer new surgical means to treat functional disorders of the GI tract. A true revolution in surgery will be initiated by cognitive surgery—the comprehensive use of computerized support." @default.
