• W2738910010 abstract "Neural tube defects (NTDs) are the second most common group of severely disabling birth defects, following congenital heart defects. A spectrum of NTDs ranges from anencephaly and cranioschisis (complete failure of neurulation) to disorders of secondary neurulation causing spina bifida occulta with minimal or no neurologic involvement. Myelomeningocele (MMC) is the most complex congenital anomaly compatible with life, and it makes up more than 98% of the “open” spinal dysraphic states. Supplementation with folic acid (4 mg/day) has been shown to prevent 72% of NTDs in women with a previously affected pregnancy. NTDs are due to failure of neurulation, which is the folding process in vertebrate embryos that occurs between seventeenth and thirtieth days of gestation. Closure of the defect within 72 hours of delivery reduces the risk of infection in the central nervous system. Immediate ventriculoperitoneal (VP) shunt is needed for infants with associated hydrocephalus. Early bladder and bowel management, along with the monitoring for decline in motor function, VP shunt malfunctioning, Arnold-Chiari II malformations, hydromyelia, tethered cord syndrome, endocrine disorders, and musculoskeletal problems, is essential in managing individuals with NTDs. Psychological and social issues need to be addressed since childhood. Survival to adulthood is now common with proper treatment. Most adolescents with MMC complete high school, and approximately 50% move on to further education. Adults with MMC must be educated about fertility, sexuality, and reproductive issues. MMC presents lifelong challenges to affected individuals, their family, and clinicians. Vigilant surveillance and education are required to prevent life-threatening events and decrease morbidity and mortality, which in turn improve the quality of life of individuals with MMC." @default.
