- W2739889476 abstract "This chapter suggests that while at present most clinical practitioners and researchers assign the leading role in the pathogenesis of ST elevation myocardial infarction (STEMI) to acute coronary thrombosis, this position has its opponents. It discusses particular problems of the treatment of STEMI, namely, urban pharmacoinvasive management as an antidote to traffic in large, metropolitan cities. In a large city such as Moscow, some STEMI patients arrive at the hospital far later than the “gold” time standard. The chapter shows the main pre- and in-hospital complications seen in STEMI patients after pre-hospital systemic thrombolytic therapy (PH-TLT). In megapolis with heavy traffic, PH-TLT allows for myocardial reperfusion significantly earlier than in-hospital percutaneous coronary intervention (PCI). In cases where the transfer of the patient to the hospital is delayed because of heavy traffic or other reasons, the chapter considers it reasonable to perform PH-TLT with subsequent in-hospital coronary angiography and eventual angioplasty of the infarct-related artery (IRA)." @default.
