- W2740874908 abstract "Background: People with diabetes are prone to labile blood sugars post-acute coronary syndrome (ACS) due to increased insulin resistance. Patients on sulfonylureas/insulin participating in the prescribed Cardiac Rehabilitation exercise program at Auckland City Hospital were noted to be at risk of significant hypoglycaemic symptoms. Methods: Capillary blood sugars were tested on all patients on sulfonylureas ± insulin who were enrolled in this program in 2016 pre and post exercise or had hypoglycaemic symptoms. Maximal changes in blood sugars per patient are reported. A two sided student T test is used to assess difference and p < 0.05 is considered significant. Results: Blood sugars dropped from an average of 13.1 (± 5.5) mmol/l to 6.7 (± 3.0) mmol/l, p = 0.001 representing a 48.8 (± 19.6) %, drop in blood sugars during the 1-hour supervised exercise session. Blood sugars fell below 4.0 mmol/l in 30% of patients, and this was associated with symptoms of hypoglycaemia. Conclusion: People with diabetes on hypoglycaemic agents in an exercise program are at high risk of large drops in blood sugar regardless of their baseline values. All units who run exercise programs post ACS need to develop safety protocols for people with diabetes on hypoglycaemic agents to closely monitor and treat drops in blood sugars." @default.
