- W2749154921 abstract "Data on the relationship between blood pressure (BP) and risk of dementia have not been consistent and may depend on age and time of BP measured relative to dementia onset. We 1.) examined whether the association of BP and subsequent risk of dementia or Alzheimer's disease (AD) depended on age; and 2.) sought to determine if the pattern of BP changes during the eight years of study was related to the subsequent development of dementia and AD in a large community–based longitudinal study of dementia and BP. We enrolled 2,356 dementia–free members of a large health maintenance organization (Group Health Cooperative, Seattle, WA) aged ≥65 years and then assessed them biennially for incident dementia and AD, diagnosed using standard criteria. Systolic (SBP) and diastolic blood pressure (DBP) were measured at baseline and at four biennial follow–ups. Cox proportional hazards models were used to estimate hazard ratios (HR) for dementia or AD associated with baseline BP in different age groups. Generalized estimating equations were used to explore BP changes prior to onset of study outcomes. Gender, education, race, and presence of APOE*4 allele were adjusted for in both models. Among younger participants (65–74 years at–study–entry), there was increased risk for dementia in those with high baseline SBP (≥160 mmHg) (HR=1.60, 1.01–2.55) and borderline high baseline DBP (80–89 mmHg) (HR=1.59, 1.07–2.35), as compared with ideal measures (<140 mmHg and <80 mmHg, respectively). The association between SBP and dementia risk was reversed with increasing age at baseline (SBP × age interaction p = 0.01). In the younger cohort, eight–year mean SBP prior to onset was significantly higher in AD than normal subjects (+5.8 mmHg, p= 0.02). Even so, there was a similar pattern of decline over time in both AD (–0.6 mmHg/year) and normal subjects (–0.8 mmHg/year, p = 0.50). Our results suggest that SBP increases risk of dementia or AD in younger elderly but not in late old age (≥ 75 years). There is no rapid SBP decline with approach of clinical dementia onset in AD patients. Early treatment of hypertension may reduce risk for dementia." @default.
