- W2753409862 abstract "Objective: To study the occurrence, clinical profile, treatment modalities and outcome of Guillain-Barre syndrome (GBS) and its variants in pediatric neurology practice. Methods: Medical files of patients with GBS admitted to the Department of Pediatrics – University Hospital - Plovdiv were studied retrospectively for a 7-year period (2010 - 2016). Diagnosis was based on the clinical, CSF and electrophysiological criteria. Classification included classical GBS and its variants. Results: Twenty one patients (aged 1y-17y) with GBS were studied. In 2016 there is an increase in the occurence of GBS: 9 cases, which represent 43% of all GBS patients for the 7-year period and 75% of all GBS, registered in the country for 2016. Four of these cases were atypical. The analysis of all 21 patients did not show any significant age, sex or season distribution difference. Antecedent event were established in 16 out of 21 patients (76%): fever (10 patients), gastrointestinal infection (4 patients) or vaccination (2 patients). Acute muscle weakness was demonstrated in all 21 patients, symmetrical in 15 and asymmetrical in 6. Sensory signs were found in 12 children, cranial nerve involvement – in 8, autonomic failure – in 6. Nine of 21 patients show atypical presentation: cranial polyneuritis (1), Miller-Fisher syndrome (2), sensory form (1), pharyngeal-cervical-brachial variant (2), AMSAN (1). There was increase in protein and IgG in cerebrospinal fluid in all patients. NCV studies showed AIDP in 20 and AMSAN in 1 patient. Three patients reached 5 on GBS disability scale, but there was no lethal case. 19 children were treated with high-dosed IVIG. All 21 patients restored independent gait untill the 40-th day. Conclusion: There was a significant increase in the occurrence of GBS during 2016 at the Pediatric Department in Plovdiv, and atypical variants increased. Outcome was good after IVIG and additional supportive treatment for all patients." @default.
