• W2754330003 abstract "Controversy continues to exist concerning the appropriate treatment of displaced intra-articular fractures of the calcaneus. Although there has been no definitive answer as to whether these fractures should be treated operatively versus nonoperatively, the current trend is certainly toward operative fixation, the most common complications of which have dealt with the surgical wound associated with an extensile exposure. This article reports on the use of small wire ring external fixation as a viable surgical alternative in the operative treatment of calcaneal fractures. The technique for indirect reduction and external fixation is described. Proposed advantages of indirect reduction and external fixation include avoidance of an extensile exposure and therefore subsequent wound complications, immediate weight bearing, as well as providing an alternative to primary subtalar fusion, which has been recommended for extensively comminuted articular fractures (Sanders type IV). In addition, a series of 31 patients with 33 displaced intra-articular calcaneal fractures (Sanders type II-IV), 11 of which were open, were treated with small wire ring fixators. A limited plantar incision with percutaneous, fluoroscopically assisted reduction of the posterior facet was also performed on most fractures. Outcome measurements including patient age, gender, social history, pre-existing medical history, mechanism of injury, associated injuries, and fracture type/classification were recorded. Length of time to surgery, operative and postoperative complications, length of hospital stay, and time to the removal of the fixator were also documented. Clinical outcome is assessed using the American Orthopaedic Foot & Ankle Society Hindfoot Score. Conclusions: indirect reduction and external fixation is a useful, sometimes preferred surgical alternative for intra-articular calcaneal fractures. Advantages include shorter time to the operating room, immediate weight bearing, fewer serious wound problems, and no residual devices. Disadvantages include technical difficulty, less aesthetic postreduction radiographic appearance, and need for secondary surgery (removal)." @default.
