- W2760649878 abstract "Single-isocenter volumetric modulated arc therapy (VMAT) is feasible for treatment of multiple brain metastases, though planning and treatment parameters have not been standardized. Automated planning and delivery via the Varian HyperArc (HA) platform will soon be clinically available. Additionally, we have previously demonstrated an independent improvement in control of brain metastases treated with Gamma Knife (GK) radiosurgery by modulating dose hotspots within the tumor. This study compares plan quality metrics, including hotspot generation, as well as delivery times for GK versus HA. Five patients with intracranial metastases who were treated at our institution using Gamma Knife Perfexion were identified. Single-isocenter non-coplanar HA comparison plans, using 3-4 arcs, were created for each patient in Eclipse, using the same targets and prescription doses as delivered by GK plans. Plan quality was assessed using V3, V5, V12 normal brain volumes, target Dmin, Dmax, conformity index (CI, V100/target volume), and gradient index (GI, V50/V100). GI and CI were compared across modalities for individual lesions by calculation of the ratio of GK CI to HA CI and GK GI to HA GI. Hotspot size and magnitude was analyzed by comparison of target V24, V27, V30, and V33. Treatment delivery times were compared by adjusting actual GK delivery times to nominal source strength, and by estimating for HA based on maximum available dose rate for standard 6 MV photons used in planning. HA delivery times do not account for MLC or gantry motion. Comparison plans were made for 19 metastatic lesions (range 1-7/patient), ranging in size from 0.04-11.4cc. Median prescription was 20Gy (range 17-21Gy). Average target Dmin and Dmax for GKS were 19.2 and 35.5Gy compared to 15.1 and 31.5Gy for HA plans. Average V3, V5, and V12 brain were 222.5, 97.6, and 21.7 for GK plans, compared with 420.1, 143.0, and 26.0 for HA plans. Hotspot parameters including V24, V27, V30, and V33 were 0.92, 0.53, 0.25, and 0.09cc for GKS versus 0.90, 0.47, 0.15, and 0.03cc for HA plans. On a lesion-by-lesion basis, the average ratio of the conformity index for the GK plan vs the corresponding HA plan was 1.13 (median 1.10, range 0.54-2.97), with the HA plan having superior conformity in most cases. For GI, the average ratio was 0.65 (median 0.69, range 0.27 to 1.00), in favor of GK plans. For GK plans, average treatment time was 90.0 min (median 74.3, range 11.8-101.1min). For HA plans, the average monitor units was 10196.4 (median 9187, range 7778-12456) with average beam on time of 17 min (median 15.3, range 12.96-21.20). Non-coplanar VMAT (HyperArc) plans offered improved conformity, similar 12 Gy isodose volumes, and substantially faster treatment times when compared with GK.
