- W2765447943 abstract "Vessel wall imaging (VWI) is a novel neuroimaging technique that offers a critical first step toward understanding intracranial vessel morphology. Previous data suggest increased brain arterial diameter is a risk factor for vascular events and relates to both cerebral large and small vessel disease; however, the pathophysiology linking cerebrovascular remodeling to vascular disease requires further research. The current study examines the relationship between intracranial artery diameter to cerebral blood flow (CBF) and cerebrovascular reactivity (CVR) among older adults. Vanderbilt Memory & Aging Project participants free of clinical stroke and heart failure with normal cognition (NC, n=74, 74±6 years) and mild cognitive impairment (MCI, n=45, 74±8 years) were studied. Intracranial artery lumen diameter was measured across internal carotid artery (ICA), anterior cerebral artery (ACA), middle cerebral artery (MCA), and basilar artery (BA) segments using VWI MRI; mean values were calculated for bilateral segments. CBF and CVR were quantified using pseudo-continuous arterial-spin labeling MRI. Linear regression models stratified by cognitive diagnosis (NC, MCI) related intracranial lumen diameter to CBF and CVR separately. Models adjusted for age, race/ethnicity, education, APOE-e4 status, Framingham Stroke Risk Profile (minus age), and corresponding regional T1 tissue volume to correct for partial volume effects. In NC, intracranial artery lumen diameter was unrelated to CBF (p>0.06) and CVR (p>0.05). In MCI, mean MCA lumen diameter related directly to temporal CBF (right, b=8.71, p=0.03); mean ACA lumen diameter related indirectly to hemispheric CVR (left, b=-3.33, p=0.04; right, b=-3.50, p=0.03) and BA lumen diameter related directly to occipital CVR (total, b=1.51, p=0.04; left, b=1.86, p=0.04). In MCI, dilated intracranial arteries relate to elevated CBF in areas traditionally affected by Alzheimer's disease as well as widespread reduction in CVR, with the exception of occipital CVR. Results support the notion that older adults with cognitive impairment may experience an altered cerebrovascular state in which compensatory dilation relates to CBF in areas at elevated risk and further relates to diminished CVR in anterior regions. These results provide novel insight into intracranial structural-functional relations that cannot be explained by shared systemic vascular risk factors or advancing age." @default.
