There is evidence to suggest that effective teams can promote patient-centered care, continuous improvement capacity, and improved staff satisfaction, yet results of large-scale primary care transformation initiatives have been mixed. The present research is a qualitative investigation of the process by which primary care practices transition to team-based care, and how the processes underpinning this transformation affect the quality of the resulting teams. We studied 12 academic primary care practices participating in a multi-year learning collaborative, interviewing 48 practice leaders and frontline staff in a variety of roles in an effort to understand the experience of practice transformation from diverse perspectives. Using thematic network analysis, we sought to elaborate the types of change processes practices engaged in to establish team based care and the impact of these change processes on the levels of success these teams experienced. We find that both functional and cultural change processes were both essential to successfully transitioning to effective teams, that they were mutually reinforcing, and that an overreliance on one or the other form of change hindered effective teaming." @default.
