- W2765609368 abstract "Amyloid-β and tau are two major pathological proteins that accumulate and spread throughout the brain in Alzheimer's disease (AD). Unlike the diffuse accumulation of amyloid-β throughout the neocortex, tau first appears in the medial temporal regions and spreads to distant cortical areas in a stepwise pattern. These characteristic spreading patterns were first observed in postmortem studies and have been supported by our previous in vivo positron emission tomography (PET) study based on regional involvement frequency. In the present study, we investigated in vivo spreading patterns and regional differences in sequential events involving amyloid-β and tau accumulation using conditional probability. We included 221 participants [67 AD, 64 amnestic mild cognitive impairment (MCI), and 90 healthy controls] who underwent two PET scans (18F-florbetaben for amyloid-β and 18F-AV-1451 for tau). We used conditional probability to model the sequential spread of amyloid-β and tau across the 25 brain regions. Tau accumulation was observed to begin in the medial temporal regions and spread upwards through the association cortices, and eventually involved the primary cortical areas. In contrast, amyloid-β accumulation began in the frontotemporal association cortices and spread to the entire neocortex, finally involving the medial temporal regions. Tau accumulation in the entorhinal cortex preceded amyloid-β accumulation in the cortical regions, and in the medial temporal cortices tau accumulation preceded amyloid-β accumulation. Conversely, in the remaining neocortex, amyloid-β always preceded tau. We found supporting evidence for amyloid-β and tau spreading patterns using conditional probability. Tau first accumulates in the entorhinal cortex before the appearance of amyloid-β in the neocortex and may spread to the neighboring medial temporal regions without amyloid-β. We hypothesize that amyloid-β plays a role in tau spreading to distant cortical areas." @default.
