- W2765655966 abstract "SESSION TITLE: Lung Pathology 1 SESSION TYPE: Affiliate Case Report Poster PRESENTED ON: Tuesday, October 31, 2017 at 01:30 PM - 02:30 PM INTRODUCTION: Pulmonary metastasis from uterine Leiomyosarcoma are rare and impose a challenge in clinical practice. We present a rare case of histologically proven metastatic Leiomysarcoma presenting as multiple lung masses. Although there is no standard therapy available, newer therapy have been shown to effective in current literature CASE PRESENTATION: A 63 y.o. female presented with multiple lung masses and lung nodules of varying size from 1.8 cm to 5.8 cm associated with symptoms of dyspnea and cough. She also had a history of bilateral Breast Cancer in 2001 and 2009 treated with lumpectomy/radiation therapy and was on tamoxifen for 5 years with no recurrence. She developed post-menopausal bleeding and had D&C which was negative for malignancy. Per patient, she had history of lung nodules for the last 2 years. Multiple biopsies were done in the past but were non-diagnostic however she had progressively growing nodules therefore she was referred to our lung nodule program. A PET scan was performed showed strongly PET AVID bilateral lung masses and lung nodules with non-specific uptake of inside lining of the uterus. CT guided biopsy of the left lung mass showed high grade Leiomyosarcoma with smooth muscle actin antibody and desmin positive with lymphatic invasion. Patient underwent total abdominal hysterectomy with salphingo-oophorectomy followed by recommendation to initiate chemotherapy. DISCUSSION: Leiomysarcoma, a rare malignant tumor of smooth muscle origin, accounting to 1% of malignancy. However it has high propensity of hematogenous spread. An aggressive tumor with poor 5-year survival rate between 15-20%. Therefore, prompt diagnosis and treatment is imperative. In patients with single metastatic lesions, surgical treatment is a recommended approach however in multiple metastatic lesions, conventional chemo although recommended has poor survival benefit. Newer studies indicate use of TS-1(tegafur, gemeracil, and oteracil potassium) has been shown to successfully control the disease with excellent tolerability. CONCLUSIONS: In conclusion, metastaic pulmonary Leiomyosarcoma although rare has a grave prognosis. Therefore prompt clinical suspicion, detection and appropriate treatment is recommended to reduce the mortality in this disease. Reference #1: Case Rep Med. 2013;2013:496304. Epub 2013 Mar 5.
