- W2766451107 abstract "Early tau-PET data have shown associations between tau and cognitive performance, including in clinically normal (CN) older adults. Recently acquired serial tau-PET data allowed us to assess the association between change in tau, change in amyloid, and change in cognition. One hundred and eleven CN (age=66–90; MMSE≥26) had baseline evaluations within three-months period [0.0–0.5y]: Neuropsychological assessment included the PACC (Preclinical Alzheimer Cognitive Composite); PiB-PET assessed amyloid; and Flortaucipir-PET assessed tau. During the next two years [1.0–2.8y], all participants had annual PACC evaluations and forty-three participants had repeated PiB- and tau-PET. PiB was measured in a large neocortical aggregate, tau in bilateral inferior temporal gyri. Partial volume effects were corrected for. Linear regressions investigated the association between PACC performance and tau at baseline. Mixed-models with random intercepts investigated the association between PACC decline and baseline or annual change in tau. Analyses were adjusted for demographics and follow-up duration. At baseline, high temporal tau was associated with lower PACC scores (Fig.1; Estimate=-0.14±0.07, p=0.048). Elevated baseline tau also predicted PACC decline in the following two years (Fig.2; Est.=-0.09±0.03, p=0.004). Rates of change were significantly different between tau (0.23±0.11z-score/y), cognition (-0.14±0.31z-score/y, p=0.027), and amyloid (0.05±0.06z-score/y, p<0.0001). Longitudinal tau accumulation in inferior temporal was associated with PACC decline (Fig.3; Est.=-1.42±0.40, p=0.0005), including after adjusting for baseline tau (p=0.053). In contrast, change in amyloid did not predict change in cognition (Fig.4; Est.=-0.32±0.85, p=0.70). Tau and PACC were significantly associated in high-PiB (baseline: p=0.043, baseline-to-change: p=0.011, change-to-change: p=0.005), but not in low-PiB individuals (all p>0.40). A marginal association was nevertheless observed in the low-PiB participants between change in cognition and change in tau in the voxels with elevated tau signal at baseline (Fig.5; Est.=-0.51±0.28, p=0.075)." @default.
