- W2766795800 abstract "Antidepressant use is common in the elderly and has been shown to improve cognition, possibly through a decrease in depressive symptoms or amyloid-β formation. However, recent studies also suggested that different classes of antidepressants might have different effects on cognition, and some could be associated with an increased risk of dementia. We studied 1234 older women (mean age 83.2 years) without cognitive impairment at baseline (2002–2004). Baseline medication use, including the use of Selective Serotonin Reuptake Inhibitors (SSRIs), tricyclic antidepressants and trazodone was assessed using an interviewer-administered questionnaire and verified by medication containers and computerized dictionary. Cognitive status (normal, mild cognitive impairment (MCI) or dementia) over the subsequent five years was adjudicated by an expert clinical panel. We used multivariable logistic regression to examine the relationship between use of different classes of antidepressants and the risk of incident MCI and dementia. Compared to women not taking any antidepressants (n=1094), those who were on SSRIs alone (n=77) or trazadone alone (n=15) were more than twice (multivariable odds ratio (MOR)= 2.53, 95% CI= 1.49–4.28) or three times (MOR= 3.56, 95% CI= 1.12–11.30) as likely to develop MCI or dementia, respectively, after adjustment for age, education, alcohol, history of stroke and coronary heart disease, benzodiazepine use and baseline cognitive status. Use of tricyclic antidepressants alone (n=32) was not associated with risk of MCI or dementia (MOR= 1.12, 95% CI= 0.52–2.40). Besides, no association was found for women who were on multiple (n=9) or other classes (n=7) of antidepressants. Further adjustment for depressive symptoms, as assessed by Geriatric Depression Scale (GDS) scores did not appreciably alter the results. Among non-demented community-dwelling older women, the use of SSRIs or trazadone alone was related to substantially increased risk of incident MCI or dementia, independent of depressive symptoms. Further studies are needed to help understand potential mechanisms. The use of antidepressants should be taken cautiously in older adults who are at high risk of cognitive impairment." @default.
