Este síndrome doloroso recibe el nombre de dolor neuropático central. Esta actualización tiene por objetivo efectuar una revisión de este síndrome complejo, repasar sus características clínicas principales, brindar una actualización sobre los conocimientos de su fisiopatogenia, así como brindar una aproximación diagnóstica y terapéutica de esta entidad. Se realizó una búsqueda de artículos científicos relacionados con dolor neuropático central en las plataformas virtuales de Pubmed, Medline, LILACS, SciELO, Medscape, y Cochrane. Para la búsqueda se utilizaron los términos: «neuropathic pain, central neuropathic pain, central pain, central neuropathic pain treatment». Los artículos fueron revisados y verificados por su contenido relevante. El dolor neuropático central posee variados mecanismos potenciales que se relacionan con las diferentes estrategias terapéuticas posibles hasta el momento. Su reconocimiento clínico precoz y las terapéuticas multimodales eficaces contribuyen sin duda a disminuir la exposición al dolor y mejorar el estado funcional en el paciente. Pain remains a complex situation within medical practice. It is necessary to consider that certain painful syndromes may be produced by the direct impairment of the central nervous system. After acute injury, the nervous system responds inappropriately to damage through multiple structural and modulation mechanisms. This painful syndrome is called central neuropathic pain. The present study aims at reviewing this complex syndrome; examining its main clinical features; providing an update on the knowledge of its pathophysiology; as well as providing a diagnostic and therapeutic approach to this entity. A search for scientific articles related to central neuropathic pain was performed on the virtual platforms Pubmed, Medline, LILACS, SciELO, Medscape, and Cochrane. The keywords used for the digital search were: “neuropathic pain, central neuropathic pain, central pain, central neuropathic pain treatment”. All the retrieved papers were reviewed and checked for content relevance. Central neuropathic pain has several potential mechanisms that are related to the different therapeutic strategies used up to the present. Undoubtedly, its early clinical recognition and effective multimodal therapies contribute to decrease pain exposure and to improve patients’ functional status." @default.
