- W2771970885 abstract "Background: There is a significant effort to increase supply of human organs for transplantation by considering donors with reported risk factors. The ever-increasing emphasis on laboratory evaluation to compensate for lack of reliable next-of-kin information necessitates better understanding of risk factors for organ donors vs. other donor types (blood, tissue and reproductive). It is common to treat organ and tissue donors as one population with similar risk factors. However, recruitment strategies and targeted populations are different. To better understand the differences in risk factors for major communicable diseases we analyzed the most recent data from our laboratory for organ, tissue, cornea and reproductive donors. We compared HBV, HCV, HIV-1 and Syphilis prevalence among prospective donors screened by our laboratory. We believe that our data is representative for the United States (US) donor population and will contribute to a better understanding of infectious disease risks factors among segments of the US population targeted for organ and tissue donations. Methods: We used data from 2012-13. We tested 18541 donors: organ (2298), tissue (3891), cornea (6822) and reproductive (5530) donors from California, Nevada, Oregon, Washington, New Mexico and Utah using HCV 3.0 EIA (Ortho Diagnostics), HIV-1/2 EIA (Bio-Rad), RPR (ASI), HBsAg (Bio-Rad), HBcAb (Ortho Diagnostics) and TMA NAT to test for HCV/HIV RNA and HBV DNA (ULTRIO, GenProbe). Results: In the table, we summarized the seroprevalence of HBV, HCV, HIV and Syphilis among donors. There were significant differences in among all groups (p<0.05 or lower). We observed very significant differences in prevalence of HCV between organ and tissue donors (p<0.001). The cornea and tissue donors are less infected by HBV, while reproductive donors are more likely to have HIV infections and HBV (p<0.01).Table: No Caption available.Conclusions: Organ donors have significantly higher risk for communicable diseases than other donors. Our data strongly supports recent CDC guidelines for organ donor testing suggesting that all organ donors should be tested by the most sensitive NAT for HCV.
