- W2772319614 abstract "This work describes a novel technique for the removal of tilted and apex embedded Bard Recovery, G2, G2 Express and Eclipse inferior vena cava filters (IVCFs) (Bard Peripheral Vascular Tempe AZ). Between March 2009 and July 2011, 191 patients presented for removal of their Bard Recovery (n=1), G2 (n=28), G2 Express (n=66), or Eclipse (n=96) IVCF. Thirty Three IVCFs(17.3%) in 32 patients were found to be severely tilted with the filter apex embedded into the caval wall. A modified loop snare technique was used to remove IVCFs with an embedded apex as follows. Through a 16Fr × 35 cm sheath (Cook, Bloomington IN), a 5Fr Contra catheter (Boston Scientific, Matick MA), a 260cm 035 Angled Glidewire (Terumo, Somerset NJ), and a 15mm or 20mm Amplatz Gooseneck snare (EV3, Plymouth MN) were used to form an in situ snare through one or more filter arms or legs. Over both ends of the looped wire, the curved metal stiffener from a LABS 200 transjugular liver biopsy set (Cook) was used to close the wire loop forcefully around the trapped filter arms and legs. The IVCF was then withdrawn back into the sheath folded over, pulling the inverted filter apex free of the caval wall. The technique was used to successfully remove all 33 filters including 1 Recovery, 10 G2, 13 G2 Express and 9 Eclipse IVCFs. The average length of implantation for the IVCFs removed with this technique was 556 days (range 11-2437 days, median 268 days). Of the 33 IVCf's removed, 13 (7.7%) were broken at the time of initial evaluation. No IVCF fracture occurred related to the removal technique. No procedure related complications occurred. We have found the described technique to be safe and effective for the removal of tilted and apex embedded Bard Recovery, G2, G2 Express and Eclipse IVCFs. Since its introduction into our practice, this technique has become the primary method for removal of tilted and apex embedded Bard IVCFs. Since then, we have not experienced a single technical failure of removal of any Bard Recovery, G2, G2 Express or Eclipse IVCF in over 191 cases. There has been no fracture of an IVCF during removal or complication during IVCF removal associated with the technique." @default.
