• W2772488690 abstract "Purpose: Understanding the immune status of individual recipients is crucial for fine-tuning immunosuppression (IS) to avoid graft rejection and infection/over-IS. We evaluated immunological monitoring tests in living related liver transplant (LTx) recipients. In order to reflect monitoring data to diagnosis and IS-adjustment, immune-assays possible to obtain results in a short period of time were applied. Methods: Eighteen (10 children, 8 adults) recipients were examined. Peripheral blood/PBMCs were collected from LTx recipients at peri-transplant period (once or twice weekly). The mean follow-up time after LTx was 67.4±34.1 days (range: 4 to 23 wks). Immunological assays including Immuknow(TM) assay and lymphocyte subset analysis (FCM) were performed. Recipient's PBMCs were co-cultured with irradiated donor PBMCs, and IFNγ ELISPOT and CD154 expression of CD4+ T cells were assessed. Results: In 14 patients, ACR was clinically diagnosed for 20 times in which 16 were biopsy proven. At the time of ACR, significant increases in the Immuknow(TM) value, IFNγ ELISPOT andpercentage of alloreactive CD154+CD4+ T cells were observed (Fig.1A). The increased values observed in more than 2 of these immunological assays were useful for predicting the ACR in 70 % of episodes, whereas that was only 29% by a single assay (Fig.1B). In 11 patients who had experienced CMV infection, Immuknow(TM) value significantly dropped at the time of infection (268±47 to 118±24 ng/ml, p=0.0012), while other assays did not display a difference. The population of effector/memory or CD4+Foxp3+ regulatory T cells did not significantly change depending on the time after surgery or clinical events including ACR or infection. Conclusion: Although a single immunological monitoring test has limitations in terms of reliability, along with general hematology and blood chemistry tests, combination of Immuknow(TM), IFNγ ELISPOT assay and Ag-specific CD4+CD154+ T cell phenotype analysis seems valuable for understanding the immune status in LTx recipients.Figure: No Caption available." @default.
