• W2777165737 abstract "[EN]The calcium-phosphorus metabolism is an important element of the overall mechanism supporting the homeostasis and is also a significant factor in the integrity and functional capability of the locomotory system. Impairments in calcium-phosphorus metabolism lead to major abnormalities in metabolic processes in bones, regulation of muscle contractions, basic functios of the excretory system and hormonal disorders with additional pleyotropic effects on numerous other organs and systems.The focus of the dissertation are the manifestations of impaired regulation of the calcium-phosphorus metabolism. Explained are the physiological defense mechanisms, as well as the reasons and deviations observed in hyper- and hypocalcaemia, hyper- and hypophosphataemia and the role of the endocrinological sytem and the kidneys in these processes.The goal of the investigation is to study the regulatory effects of analogs of vitamin D on calcium-phosphorus metabolism in renal function replacement in end-stage renal disease. To achieve this goal selected parameters of the calcium-phosphorus metabolism are studied under the influence of different vitamin D analogs. Also studied are some acute phase markers of inflammation. A stable and reproducible model of missing or negligibly low synthesis of vitamin D is selected while the study is carried out for people with end-stage renal disease, on hemodialysis due to the necessary renal function replacement. Results are interpreted on the background of the achievements of leading researchers in the area.//////////////////////////////////////[BG]Калциево-фосфорната обмяна е важен елемент от цялостния механизъм за поддържане на хомеостазата, а също така е и съществен фактор в интегритета и функционалната годност на опорно-двигателния апарат. Нарушенията й водят до тежки отклонения в обменните процеси в костната система, регулацията на мускулните съкращения, основните функции на отделителната система, до хормонални смущения с допълнителни плейотропни ефекти върху редица други органи и системи. Обект на настоящия дисертационен труд са проявите на нарушената регулация на калциево-фосфорната обмяна. Изясняват се физиологичните защитни механизми, както и причините и отклоненията при хипер- и хипокалциемия, хипер- и хипофосфатемия, както и ролята на ендокринната система и бъбреците в тези процеси. Цел на изследването е да се проучат регулаторните ефекти на аналози на витамин D върху калциево-фосфорната обмяна при заместване на бъбречната функция в стадий 5 на хроничното бъбречно заболяване. За постигане на тази цел се проследяват подбрани параметри на калциево-фосфорната обмяна при въздействие с различни витамин D аналози. Изследвани са и някои острофазови маркери на възпалението. Подбран е стабилен репродуцируем модел на липсваща или пренебрежимо ниска синтеза на витамин D, като проучването е проведено при лица с 5-ти стадий на хронично бъбречно заболяване, на хемодиализа, поради налагащото се заместване на бъбречната функция.
