- W2777917609 abstract "In COPD, significant small airway disease can occur before it becomes detectible via traditional spirometric measures, suggesting that new methods are required to identify these changes. In this study we assessed lung function in patients with COPD using a new method which utilises the inspired sinewave technique (IST). We aim to assess the repeatability of the IST device in COPD patients and volunteers and compare IST measures with those obtained from standard pulmonary function tests (PFTs). The IST device functions by injecting a tracer gas (N2O) into inspired air, the concentration of which oscillates sinusoidally with a predetermined period. Based upon a mathematical single-compartment tidal-ventilation lung model, the differences in amplitude/phase of the inspired and expired N2O sinewaves enable values to be calculated for dead space (VD), functional residual capacity (FRC), pulmonary blood flow (QP), and an index for ventilation inhomogeneity. 50 COPD patients and 20 healthy volunteers performed PFTs (spirometry, body plethysmography; BP, gas transfer), and repeated IST tests - consisting of normal tidal breathing performed with different N2O sinewave periods. The parameters measured on healthy participants using the IST device were highly repeatable with coefficients of variation (CV) for VD, FRC and QP ≈4-10%, and for COPD patients CVs were higher at >10%. Early results demonstrate that COPD patients had a greater period dependency in FRCIST and a lower FRCIST/FRCBP ratio than healthy volunteers (0.33 vs. 0.61 respectively), which was related to COPD severity (FEV1%pred ≥ 80% = 0.41, 50-79% = 0.35, 30-49% = 0.31," @default.
