• W2779196494 abstract "<b>Background:</b> Asthma is one of the most prevalent respiratory diseases in the world. Many individuals with mild-moderate asthma have characteristics of a more serious disease. <b>Aims:</b> Determine the risk factors for hospitalization and ICU admission in individuals with near-fatal asthma (NFA) in the emergency department (ED). <b>Methods:</b> Analytical cross-sectional survey of patients (n=217) who visited the ED for acute asthma during January 2017. We identified asthma exacerbations as defined by GINA report. <b>Results:</b> Women (67%) were more common in ED, the mean age was 41.5 ± 15.2 years. Up to 79% of patients visited an ED in the last year and winter was the worst season for disease control in 96%. The 34% of patients consumed systemic steroids and 27% used >12 vials of salbutamol in the last year. The risk factos for hospitalization and ICU admission were: smoking history of ≥10 pack-years (OR 3.39, 95% CI 1.18 - 9.74, p=0.023); previous use of mechanical ventilation (OR 50, 95% CI 12.9 – 194.3, p=<0.001); history of atopy (OR 3.24, 95% CI 1.44 – 7.29, p=0.004); a previous diagnosis of NFA (OR 7.15, 95% CI 2.36 – 21.6, p=<0.001). Other characteristics non-significantly increased this risk: the lack of treatment with ED visits for acute control; biomass smoke exposure of 50 hour-years and obesity. Poor adherence to medical treatment was present in 62%, only 8.3% had an action plan and 2.3% knew the SMART strategy. <b>Conclusions:</b> Risk factors for NFA are preventable factors of asthma deaths. The role of patient education and identification of characteristics which define a more severe disease can improve the outcomes of patients." @default.
