- W2782051220 abstract "Counting of fetal movements (FM) by the mother is considered an inexpensive yet potentially efficacious screening test to prevent adverse perinatal outcomes. However considerable variation exists in FM among pregnancies. We have explored whether variation exists within pregnancies as individual variation may translate into different instructions on threshold FM to call for adjunctive testing. Prospective cohort study of all women with risk factors for fetal death undergoing fetal testing via weekly modified biophysical profile started at 36 weeks. Excluded were multiple gestations, decreased fetal movements, delivery indicated at 37 weeks, severe maternal medical conditions or severe fetal anomalies. Risk of stillbirth in last month of pregnancy was explained and women were taught in a consistent way on FM with recording of time to the 10th kick. Log sheets were verified weekly at the time of fetal testing, when instructions on FM were reinforced by dedicated nurses. The individual “baseline” time to the 10th kick based on the initial 3 days of recording was compared with the average time during the following interval until delivery for each compliant patient using Wilcoxon Signed-Ranks Test. Among 91 compliant women, there was no significant difference between time-to-the 10th kick in the initial 3 days vs the remainder of the pregnancy (P= 0.51) (figure). Only in 5/91 women (5.5%) was the average time in the remainder of the pregnancy >2 MoM above the average time in initial 3 days. 1. There is negligible within-pregnancy variation in FM, while there is significant inter-pregnancy variation. 2. In 95% of women, the time to the 10th kick during the initial 3 days establishes the pattern of FM for the remainder of the pregnancy. Standard recommendations to call the provider for failure to reach the 10th FM within 2 hours may need to be customized to individual patients." @default.
