- W2783222394 abstract "A 71-year-old woman presented with pain and hand movements vision in her left eye, following a failed XEN45 implant (Allergan, Dublin, Ireland) 11 months previously. She had a 1-mm hypopyon (Fig 1A) and an exposed XEN45 implant (Fig 1B, arrow) that stained with fluorescein (Fig 1C, arrow). She improved with intravitreal and topical antibiotics. Erosion of XEN implants through the conjunctiva can lead to severe infections such as endophthalmitis. Patients with flat blebs may have a higher risk due to direct contact of the implant and the overlying conjunctiva causing erosion, as in our case. We recommend regular reviews of XEN patients, including those with failed surgery. (Magnified version of Fig 1A-C is available online at www.aaojournal.org.) Although blebitis and endophthalmitis have been reported in patients with functioning XEN implants,2 the current case suggests a risk for infection even after failed surgery.1 It is not currently possible to quantify the population risk of erosion or endophthalmitis in patients with failed XEN surgery and further robust postmarketing surveillance of these rare but potentially devastating complications is warranted.
