• W2783298637 abstract "In 2007, the Centers for Medicare and Medicaid Systems (CMS) ushered in an era of performance measurement with its Physician Quality Reporting Initiative, later named the “Physician Quality Reporting System” (PQRS), a program that offered financial incentives to qualified providers to report quality measures to the CMS. In anticipation of the need for quality measures, the American Academy of Ophthalmology supported and worked with the American Medical Association's Physician Consortium for Performance Improvement to convene the Eye Care Workgroup to develop performance measures on which providers could report. In 2006, the workgroup developed 8 process measures, of which 4 were endorsed by the National Quality Forum and approved by the CMS for the Physician Quality Reporting Initiative program. In 2007, the Eye Care Workgroup created 5 additional measures, including 2 cataract surgery outcome measures that were endorsed by the National Quality Forum and approved by the CMS for the PQRS program (Table S1, available at www.aaojournal.org). These measures were approved by the CMS for quality reporting since the program's inception, so ophthalmologists could report on these measures to earn incentives or avoid penalties. Since that time, the measure-development workgroups convened by the Academy have created another 29 subspecialty outcome measures plus 2 appropriate-use measures. In 2014, eligible providers who successfully submitted quality data under PQRS were entitled to a 0.5% payment incentive on services covered under the Part B Medicare Physician Fee Schedule, and a 2% negative payment adjustment was applied to those who failed to meet the criteria for success. In 2015 and 2016, the positive incentive was eliminated and the negative payment adjustment remained at 2%, but an additional 2% to 4% negative adjustment associated with a value-based modifier payment was applicable, depending on the size of the practice.
