- W2783725029 abstract "Question: A 45-year-old man with no significant medical history except for vague intermittent abdominal pain was referred for an endoscopic second opinion after previous colonoscopy had been pertinent for multiple segmental submucosal lesions. Previous mucosal histopathology was nonspecific and, computed tomography was considered to exhibit uncomplicated diverticulosis. Repeat ileocolonoscopy revealed multiple smooth submucosal lesions confined to an 8-cm segment of the transverse colon (Figure A). A mucosal biopsy with large-bore forceps gave a firm elastic consistency with a positive cushion sign (Figure B). Next, on water immersion conditions, partly blood-tinged gas bubbles were seen arising from a lesion after puncture with a standard injection needle (Figure C, D). In the same line, during puncture of another lesion gas could be aspired resulting in complete collapse (Figure E, F). Pneumatosis cystoides coli (PCC) represents a rare entity of variable clinical significance, pathologically characterized by submucosal or subserosal gas-filled cysts. On endoscopy, most patients present with submucosal tumor-like lesions. Different endoscopic techniques have been reported in order to unequivocally diagnose PCC by endoscopy. A review of the external computed tomography demonstrated multiple intramural gas inclusions, a typical finding sometimes mixed up with bland diverticulosis (Figure H). In this line, many radiologists and clinicians are not familiar with the concept of benign PCC. The greatest clinical significance usually lies in the tight association with intestinal ischemia in emergency populations.
