- W2789873399 abstract "It is a fact that during recent decades, regional anesthesia has become tremendously popular in many operative settings as well as in pain therapy. Applied alone or in combination with general anesthesia, these techniques provide obvious clinical benefits, especially for high-risk surgical patients, and have repeatedly been proven to be predictable and safe to a large extent. Without a doubt, the recent introduction of ultrasound guidance has strongly contributed to these impressive advances. However, pitfalls and serious complications associated with the practice of local and regional anesthesia still occur in rare cases; therefore, it is even more essential for practitioners to have a profound knowledge of the pathophysiology, symptoms, and treatment of these adverse events. Dr Brendan T. Finucane, an internationally renowned expert in this field, was the first to publish a high-profile textbook on this topic 17 years ago. The extended third edition of Complications of Regional Anesthesia was recently released in both hardcover and eBook formats after a thorough and complete revision, featuring a newly added subtitle, Principles of Safe Practice in Local and Regional Anesthesia, with Ban C. H. Tsui as coeditor and 67 international contributors. Compared with previous editions, significant changes and adjustments have been made, and the number of chapters has been expanded from 24 to 35. The book is now divided into 7 parts, beginning with the comprehensive part titled “General Considerations.” This first part includes chapters on the history of regional anesthesia, basic safety issues (including the prevention and management of local anesthetic toxicity), and outcome studies comparing regional and general anesthesia, all of which are well worth reading. Part II addresses “Special Considerations” and contains, without exception, topics of fundamental importance for clinical practice. Following the chapters describing the issues of regional anesthesia in the presence of neurological disease and preexisting anticoagulant therapy, respectively, both the mechanisms and management of nerve injuries as well as infections associated with regional anesthesia are comprehensively discussed. A chapter on the practical aspects of continuous nerve blocks thematically rounds off the second part of the book. The third part focuses on nerve blocks involving anatomic regions of the body (including neuraxial approaches) and mainly addresses block-specific safety issues as well as the management of adverse events. Unfortunately, neither all types of modern blocks (eg, fascial plane techniques) nor recent modifications to existing techniques have been considered in detail. Analogously, in part IV, these issues are discussed with specific regard to different patient populations. Here the book provides a comprehensive collection of reviews on the safe practice of local and regional anesthesia in children and the elderly, in obstetric and obese patients, and in those patients with chronic pain. The fifth part, “Special Environments: Safe Practice and Management of Adverse Events,” impressively documents the widespread use of locoregional procedures in disciplines other than anesthesiology: dentists, ophthalmologists, plastic and orthopedic surgeons, and emergency room physicians—all notable practitioners with profound clinical expertise—critically evaluate the importance and safety of local and regional anesthesia in their specific field of practice. The results of actual morbidity studies are both summarized and discussed in part VI. Authors from North America (the United States and Canada), Australia, and Europe (the United Kingdom, France, and Nordic countries) present their “local” data, and this part offers both a unique and comprehensive international overview on the morbidity and mortality data associated with locoregional procedures as a result. The final part of the book is dedicated to a discussion of the medicolegal issues specific to local and regional anesthesia. Overall, I want to compliment both the editors and authors on the thoroughly revised third edition of Complications of Regional Anesthesia. The main title appears to be rather misleading, in my opinion, as it may give the reader the impression that the book only focuses on complications and thus aims to discredit the widespread use of regional anesthesia. In fact, the exact opposite is true: this book provides a unique and timely overview of this rare but highly relevant topic, and it presents clear and comprehensive principles of safe practice. It is consistently written based on the maxim that only when the mechanisms of adverse events and complications are understood in detail can they be effectively diagnosed, treated, and avoided. Consequently, this book is essential for physicians of all disciplines who regularly apply local and regional anesthesia in their daily practice, and it will definitely contribute to further improving the safety of these techniques.
