- W2794758413 abstract "To the Editor: Kudos to Levinson and for their recent Commentary on the need for political activism.1 That it was written by medical students is noteworthy; however, it was a bit slanted. True advocates must inform their opinions from both sides before drawing a conclusion. The United States was founded on divergent opinions, and freedoms are codified in the Bill of Rights. Yet, in this hyperpartisan environment, those who do not agree with the more vocal are demeaned, harassed, and attacked—regardless of truth. Conservative speakers are threatened with riots and mobs. Some leaders of host institutions abdicate their right to free speech by claiming they cannot provide adequate security. Yet, when Iranian President Ahmadinejad was invited to New York’s Columbia University, he spoke of hatred and lies for two hours, and the school protected him amid protests.2 A tragedy is unfolding—one that advocacy can and should reverse—the forfeiture of a foundational freedom. I applaud the authors for participating in the health care reform debate, but has the Affordable Care Act (ACA) really lived up to its expectations? It expanded Medicaid, yes, but what about the insurance expense for others? Premiums have skyrocketed, and insurers have led a mass exodus out of exchanges, leaving some counties and states with only one option. The most recent data show that the number of uninsured dropped from 48.6 million (2010) to 28.6 million (2016)3—improvement, but hardly universal coverage as touted. Recall President Obama’s claims that “If you like your doctor, you can keep your doctor. If you like your health care plan, you can keep your health care plan.”4 Neither was true. We medical providers should have a voice in health care reform. Who better to offer solutions? We must work with government to address the failings of the ACA, whether to repeal and replace it, build upon it, or move in an entirely new direction. Effective negotiations are best accomplished by rising above sensational partisan sound bites and focusing on the facts. To foster and support advocacy, medical education must address both sides. Divergent viewpoints must be encouraged, given a forum, and respected. If we can instill this sliver of professionalism and common courtesy into the next generation of physicians, then perhaps we will have been the greatest advocates of all. Joshua D.
