We hypothesized that men with severe oligospermia will take longer to improve semen parameters due to greater degree of testicular failure compared to men with oligospermia. METHODS We identified men with severe oligospermia who had microscopic subinguinal varicocelectomy between 2015 to April 2017. We compared outcomes of semen parameters between men with severe oligospermia (<5million/cc) and oligospermia (5 - 15million/cc) confirmed on at least 2 semen analyses. Follow up semen analysis was obtained at 3 and 6 months. We excluded men with azoospermia or cryptozoospermia. Data were analyzed using Microsoft excel, p<0.05 was considered statistically significant. RESULTS A total of 66 men underwent microscopic sub inguinal varicocelectomy between May 2015 to April 2017 for infertility. Of the 66 men, 27 had severe oligospermia and 39 had oligospermia. The total motile sperm count improved in both men with oligospermia (17.2 to 23.5 million) and severe oligospermia at 3 months (1.8 to 9.1 million, p < 0.05). Of note, when we evaluated TMSC at 6 months for the men with severe oligospermia (7.0 million) and oligospermia (24.0 million), there was no continued improvement from 3 to 6 months. Strengths of the study include consecutive men who underwent varicocele repair by a single surgeon who had semen analysis at both 3 and 6 months. Limitations of the study include small sample size and retrospective study. CONCLUSIONS Despite a greater degree of testicular failure in men with severe oligospermia, sperm parameters improve by 3 months after varicocele repair and then do not improve further.
