Gatev was calculated. To quantify the consumption of medications, the calculated daily dose (PDD) was used.Results. Comparative analysis of antibacterial therapy of salpingoophoritis in hospital before and after the introduction of the “Order of medical care for the profile of «obstetrics and gynecology...”. Leading groups of antibacterial drugs were identified.   А comparative analysis of the frequency of assignment of various INN was made. The К. Gate’s coefficient was 0.281. The calculation of the average prescribed daily doses of antibacterial drugs was performed.Conclusion. The review of the consumption of antibacterial drugs for salpingoophoritis therapy in hospital settings before and after the introduction of the “Order of medical care for the profile “obstetrics and gynecology...”, showed that the range of antibacterial drugs used increased from 18 to 24 INN, mainly due to the inclusion of modern medicines. In the structure of appointments in 2013–2014, the proportion of drugs not recommended for use by the «Order of medical care for the profile of «obstetrics and gynecology» was 37.5%. It was found that PDD clarithromycin, clindamycin and cefotaxime did not meet the recommended dosing regimens specified in the clinical recommendations for antibiotic therapy. The gap between actual clinical practice and existing clinical recommendations determines the need for in-depth pharmacoepidemiological evaluation to identify the causes and develop measures of an organizational and educational nature to optimize the use of medications. Цель работы – проанализировать потребление антибактериальных препаратов при терапии сальпингоофорита (СО) в условиях стационара и оценить изменения реальной клинической практики антибиотикотерапии после внедрения «Порядка оказания медицинской помощи по профилю “акушерство и гинекология” (за исключением использования вспомогательных репродуктивных технологий)» (далее «Порядок оказания медицинской помощи...»).Материалы и методы. В ретроспективном исследовании проведена оценка назначений антибактериальных препаратов при терапии сальпингоофорита в специализированных лечебно-профилактических учреждениях г. Томска в период 2010–2014 гг. Проанализировано 160 историй болезни. Структура назначений оценивалась на основе применения анатомо-терапевтическо-химической классификации и международных непатентованных наименований (МНН), рассчитывался коэффициент структурных сдвигов К. Гатева. Для количественной оценки потребления лекарственных препаратов использовался расчет назначенной суточной дозы (рrescribed daily dose (РDD)).Результаты. Выполнен сравнительный анализ антибактериальной терапии СО в условиях стационара до и после введения «Порядка оказания медицинской помощи...». Выявлены лидирующие группы антибактериальных препаратов. Проведен сравнительный анализ частоты назначения МНН. Рассчитан коэффициент К. Гатева, который составил 0,281. Выполнен расчет средних назначенных суточных доз антибактериальных препаратов.Заключение. Обзор потребления антибактериальных препаратов для терапии СО в условиях стационара до и после введения «Порядка оказания медицинской помощи...» показал, что ассортимент применяемых антибактериальных препаратов увеличился с 18 до 24 МНН, преимущественно за счет включения современных лекарственных препаратов. Тем не менее в структуре назначений в 2013–3014 гг. доля препаратов, не рекомендованных к использованию «Порядком оказания медицинской помощи», составила 37,5%. Обнаружено, что PDD кларитромицина, клиндамицина и цефотаксима не соответствовали рекомендуемым режимам дозирования, указанным в клинических рекомендациях по антибактериальной терапии. Обнаруженный разрыв между реальной клинической практикой и существующими клиническими рекомендациями определяет необходимость углубленной фармакоэпидемиологической оценки с целью выявления причин и разработки мер организационного и образовательного характера для оптимизации использования лекарственных препаратов." @default.
