- W2800931531 abstract "Association between patient perception of their doctor and COPD-related hospitalizations was assessed. We conducted a retrospective cohort study using 2007 to 2012 Medicare Current Beneficiary Survey (MCBS) data. Sample inclusion criteria were COPD diagnosis, continuous Medicare Parts A and B coverage, and having a usual source of care. A patient perception of their doctor index, with scores ranging from 12 to 48, was created using 12 MCBS survey items. Perception scores ≥ the median score were categorized as positive doctor perceptions. Individuals were excluded if they had missing data on any items in the doctor perception index, had a proxy responder, or had Alzheimer’s disease or dementia. COPD-related hospitalizations were identified based on primary or secondary ICD-9-CM diagnosis code for COPD. Patient perception of their doctor and hospitalization were assessed during the calendar year following COPD diagnosis identification in the prior year. SAS 9.4 and STATA 12.1 for Linux and an alpha of 0.05 were used in all analysis. A multivariate logistic model assessed association between patient perception of their doctor and risk of having any COPD-related hospitalization. A multivariate generalized linear model assessed association between patient perception of their doctor and number of COPD-related hospitalizations. Both models adjusted for covariates selected from stepwise procedures. Among 2,306 individuals who met study criteria, 52% were 75 years or older, 54% were women, and 86% were white. In overall sample analysis, no association was found between patient perception of their doctor and COPD-related hospitalizations. However, in subgroup analysis, oxygen users without positive doctor perceptions were more likely to have a COPD-related hospitalization (odds ratio=1.73, p=0.010) and had more COPD-related hospitalizations (average marginal effect=0.155, p=0.028) as compared to those who had positive doctor perceptions. The findings indicate potential importance of patient-doctor relationship on patient outcome in severe COPD patients." @default.
