- W2801471067 abstract "Ventricular tachycardia (VT) is an important cause of morbidity and mortality in patients with heart disease. Implantable cardioverter defibrillators (ICDs) have proven efficacy in terminating ventricular arrhythmias either by antitachycardia pacing or a shock, or reducing the risk of sudden cardiac death. Unfortunately, these life-saving therapies do not prevent arrhythmia recurrences, and shocks are painful, leading to diminished quality of life. Posttraumatic stress disorder is common in patients who receive ICD shocks. In addition, arrhythmia episodes are associated with an increased risk of subsequent heart failure and death, which may simply be a marker of deteriorating cardiac status, but there is concern that shocks for arrhythmia episodes may also contribute to cardiac deterioration. There is, as yet, no convincing evidence that preventing VT episodes prolongs survival and, thus, we are not compelled to treat patients who have rare episodes of VT that are asymptomatic and terminated by antitachycardia pacing. When VT causes serious symptoms, however, and particularly when frequent, therapy to prevent episodes is important.Randomized trials have demonstrated the efficacy of both sotalol and amiodarone in reducing VT recurrences. Risks of proarrhythmia from QT prolongation, negative inotropic effects, and a renal route of excretion limit sotalol use. Amiodarone is more effective, but has multiple organ toxicities and has been associated with increased mortality in class III heart failure. In patients with ICDs, there are other important considerations for drug therapy (Figure).
