- W2801686500 abstract "Purpose: Osteoarthritis (OA) is one of the most prevalent diseases in the elderly with high economic burden for health care systems worldwide. Pathophysiology is characterized by degradation of extracellular matrix components (ECM), eventually leading to progressive loss of cartilage and bone. Therapeutic strategies are limited to treating major symptoms during onset and progression of the disease with joint arthroplasty being the ultimate treatment in end-stage OA. Approaches to halt disease progression are not existent. In preliminary studies we demonstrated an influence of T cells on MMP/ADAMTS secretion. The aim of this project was to quantitatively analyze the influence of pro-/anti-inflammatory macrophage phenotype (M1/M2) on enzymatic, chondrodestructive processes in a direct co-culture. Methods: During arthroplastic surgery SM, chondrocytes (CHO) and peripheral blood (PB) where obtained from a total of 28 patients (age: 65.9 ± 11.1 yrs; 16m, 12f) with moderate to severe osteoarthritis of the knee (Kellgren & Lawrence grade II-IV). CHO were isolated from cartilage by enzymatic. Subsequently CHO were filtered and taken into cell culture for 9 days. Simultaneously autologous mononuclear cells (MNC) were isolated from PB via density gradient centrifugation. After centrifugation, MNC were depleted and native macrophages were differentiated into pro-inflammatory M1 (CD14+CD80+HLA-DR+) or anti-inflammatory M2 (CD14+CD163+CD206+), for 9 days according to manufacturer's protocol (Promocell, Germany). Purity of macrophage differentiation was confirmed by flow cytometry. CHO/M1 and CHO/M2 co-cultures (adding 10ng/ml GM-CSF, resp. 10ng/ml M-CSF) were further cultivated for 5 more days. As a control CHO and M1/M2 monocultures were cultivated in the same medium. The continued macrophage differentiation was once again controlled by flow cytometry after 5 days. The concentration of chondro-destructive MMP-1/-3/-9; ADAMTS-5 in each mono- or co-culture was measured with ELISA after the final time of incubation. Furthermore, MNC were isolated from SM by density gradient centrifugation, magnetic cell sorting and enzymatic digestion. The relative amount of pro- and anti-inflammatory macrophages was determined by flow cytometry: M1 (CD14+CD80+HLA-DR+); M2 (CD14+CD163+CD206+). Results: After 5 days of incubation in direct cell culture with autologous chondrocytes, M1/M2 widely obtained their phenotype, as proven by M1/M2 purity of 90±2.5%, resp. 87.3±3.0% at day 5 compared to 98.2±1.2%/81.2±2.4 at day 0. Both M1 and M2 generated a highly significant induction of MMP-1/-3 and ADAMTS-5-secretion in chondrocyte co-culture compared to chondrocyte and macrophage-monocultures, with M2 having the significantly strongest effect on MMP1 and ADAMTS5, whereas M1 enhanced MMP3-secretion significantly most. The highest values for MMP9 were measured in macrophage monocultures. Conclusions: This is to our knowledge the first experiment to achieve stable macrophage phenotypes for the complete cell culture duration. We show here, that M2 as well as M1 macrophages increase enzymatic secretion in co-culture with chondrocytes significantly. Our data suggests that macrophages play an important role in the process of enzymatic cartilage degradation during disease progression, although our data neither clearly indicates which phenotype has the predominant influence, nor if M1/M2 act on different pathways. We show here that macrophages act extremely versatile and can hardly be squeezed into the conventional pro-/anti-inflammatory pattern." @default.
