Atrial fibrillation is the underlying rhythm, often associated with longstanding and severe mitral stenosis. In patients in whom the pulmonary capillary wedge pressure is used to measure mitral valve gradients, phase shift the peak of the V wave to the downslope of the left ventricular pressure before planimetering the gradient area. The use of echo-Doppler and flow velocity in determination of the mitral valve area is the hemodynamic standard, developed by validation from invasive hemodynamic methods used in calculating valve area. Pulsus alternans is an unusual phenomenon in mitral valve disease and especially in mitral stenosis. A two-dimensional echocardiogram revealed thickened bioprosthetic mitral valve leaflets and moderate to severe mitral stenosis, with an estimated valve area of 1.3 cm2 without valvular regurgitation." @default.
