• W2804634178 abstract "Surgical excision of large malignant lower eyelid tumors may cause important full-thickness eyelid defects. The reconstruction of such defects must restore the physiologic function of the eyelid and also re-establish an acceptable aesthetic result. We report the outcomes of full-thickness excision of tumors extending over half of the horizontal lid length, followed by reconstruction using a nasal chondromucosal graft (coming from the ipsilateral ala of the nose) and an upper eyelid myocutaneous flap. Histological analysis of the specimen identified the tumor type and surgical margins for each patient. A total of 25 patients were operated using this reconstruction technique between March 2009 and June 2015: 17 basal cell carcinomas, 3 spindle cell carcinomas and 5 conjunctival melanomas (out of which 2 were associated with lentigo maligna). Mean duration of follow-up after surgery was respectively 36, 41 and 17 months for each of these 3 tumor types. We found a single local tumor recurrence and this was a basal cell carcinoma in a xeroderma pigmentosum patient. After surgery, none of the patients had lagophthalmos or ocular surface complications. Only 4 patients had a 1 mm scleral show postoperatively; 3 other patients developed a small retraction of the eyelid after adjuvant radiotherapy and a 1 mm scleral show occurred. In malignant tumors, complete surgical excision with histological margins adapted to tumor type prevents local recurrence in most cases. Our repair strategy of nasal chondromucosal graft and skin-muscle flap for large inferior eyelid defects provides good functional and aesthetic results. L’exérèse complète de volumineuses tumeurs malignes de la paupière inférieure peut entraîner une perte de substance conséquente. La reconstruction doit permettre d’obtenir à la fois un bon résultat fonctionnel et un résultat esthétique acceptable pour le patient. Nous rapportons les résultats de l’exérèse transfixiante de tumeurs occupant plus de la moitié de la paupière inférieure suivie d’une reconstruction par un greffon chondro-muqueux prélevé au niveau de l’aile du nez homolatérale et un lambeau musculocutané de dermatochalasis de la paupière supérieure. L’analyse histologique de la pièce opératoire a permis de déterminer pour chaque patient le type tumoral et les marges d’exérèse. Au total, 25 patients ont été opérés avec cette technique sur la période mars 2009–juin 2015 : 17 carcinomes basocellulaires, 3 carcinomes épidermoïdes et 5 mélanomes de la conjonctive (dont 2 étaient associés à une mélanose de Dubreuilh). Les durées moyennes de suivi post-thérapeutique ont été respectivement de 36, 41 et 17 mois pour ces trois pathologies. Sur le plan carcinologique, un seul patient a présenté une récidive au cours du suivi : il s’agissait d’un carcinome basocellulaire chez un patient présentant un xeroderma pigmentosum. En postopératoire, aucun patient n’a présenté de lagophtalmie ni de complications au niveau de la surface oculaire. Seulement 4 patients ont eu un scleral show après la chirurgie et 3 patients ont présenté une légère rétraction palpébrale inférieure due à la radiothérapie adjuvante (avec l’apparition d’un scleral show d’environ 1 mm). En cas de tumeur maligne, l’exérèse chirurgicale complète avec des marges adaptées au type histologique permet d’éviter la survenue d’une récidive locale dans la plupart des cas. La technique chirurgicale associant un greffon chondro-muqueux nasal et un lambeau musculocutané donne des résultats fonctionnels et esthétiques satisfaisants pour la reconstruction de larges pertes de substance de la paupière inférieure." @default.
