A subset of those with SDB have concurrent symptoms of excessive daytime sleepiness directly attributable to their nocturnal breathing disorder and are classified as having obstructive sleep apnea/hypopnea syndrome (2–4% of the middle-aged population). There is strong evidence for the association of obstructive sleep apnea syndrome with cardiovascular and cerebrovascular morbidity, and as adverse public health consequences are well regarded. Treatment and diagnosis have remained largely unchanged over the past 25 years. In moderate-to-severe obstructive sleep apnea/hypopnea syndrome, treatment with continuous positive airway pressure has been shown to be effective. Questions remain as to how to best screen patients with SDB; should time-consuming diagnostic procedures with high sensitivity and specificity be employed, or should simpler methods be applied for screening populations at risk (e.g., in the primary-care sector)?
