- W2805584782 abstract "Aims & Objectives: Terlipressin is shown to decrease serum creatinine, increase urine output and cause reversal of hepatorenal syndrome(in adults). There is a marked paucity of literature on the use of terlipressin in children. We looked at the efficacy (hemodynamic, biochemical response and urine output) and safety profile of terlipressin in paediatric patients with liver disease Methods Children with liver disease admitted to the PICU between 2010–2017 at Kings College Hospital, London, who required terlipressin were studied. Results 22 patient-events were evaluated in 17 patients. Twelve of these episodes were oesophageal variceal bleeding, 5 were in context of hepatorenal syndrome and 10 were post-transplant. In many patients, terlipressin was adminsitered alongside noradrenaline infusion. They all received bolus dose at the start of treatment. After start of terlipressin, elevation of mean arterial pressure was observed, an effect which was sustained even at 6 hours[median(IQR)(mmHg); 0 hrs:61(53–68); 1 hr:68(59–79); 3 hrs:65.5(57.5–80.2); 6 hrs:63(53.7–77);12 hrs:66(56.5–75.2)]. There was an transient increase in urine output which was observed at 3 hours [median (IQR): 0 hrs: 0.055(0–0.575); 3 hrs:0.8 (0–1.65)]. There was a concomitant transient elevation of lactate observed at 1 and 3 hours which settled down later. Serum creatinine decreased [median(IQR): 0 hrs:67(36.5–97):64(36–120);24 hrs:61(32.75–100.25)]. Sodium disturbances were minimal. 2 patients each experienced vomiting blanching/blueness of extremities observed. There were no episodes of gastric or coronary ischaemia. Conclusions Terlipressin can be a useful adjunctive therapy in paediatric patients with liver disease. However, it is important to keep a close eye on the microvascular and other side-effects." @default.
