- W2805800539 abstract "Bleeding from sphincterotomy site during or after endoscopic retrograde cholangiopancreatography (ERCP) is a common complication, occurring up to 15% of all ERCP with biliary sphincterotomy. Endoscopic hemostasis can be challenging as apposition of tissue during a mechanical hemostasis attempt could negate the initial intent of sphincterotomy, while application of electrocautery near the pancreatic duct orifice during a thermal hemostatic attempt could trigger inflammatory cascade. Biliary stent has been used with variable degree of success as it not only offers mechanical tamponade, but also maintains luminal patency during hemostatic endeavor. We sought to assess the safety and efficacy of fully covered self-expandable metallic stent (FCSEMS) in the management of post sphincterotomy bleeding. A retrospective cohort study was conducted using a registry of consecutive cases of endoscopic retrograde cholangiopancreatography (ERCP) with biliary FCSEMS placement at the Cleveland Clinic. Electronic medical records were utilized to collect data on patient demographics, procedure indication, endoscopic findings/intervention and peri-procedural clinical outcomes. The primary outcome of interest was the rate of clinical success, defined as successful hemostasis with the use of FCSEMS without recurrence of bleeding from sphincterotomy site within 30 days. Other outcomes of interest included technical success rate and types and rates of adverse outcome. The study was approved by IRB of the Cleveland Clinic 31 patients received FCSEMS during their ERCP for the purpose of hemostasis between 2013 and 2017. 27 patients (87%) met the definition of clinical success defined above. 4 patients required additional intervention (endoscopic or radiological) to achieve hemostasis. Technical success rate was 100%. The mean duration of stent insertion was 43 days. The most common indication for ERCP was symptomatic choledocholithiasis (18 patients), followed by asymptomatic abnormal imaging of bile duct with elevated liver function test (8 patients). 7 patients suffered adverse outcomes: 6 patients (19.3%) with post procedure pancreatitis, and one patient with ascending cholangitis. There were 6 cases of stent migration, of which 3 (9.7%) were deemed clinically significant migration resulting in inadequate hemostasis. This is the largest study to date reporting efficacy of FCSEMS in achieving meaningful hemostasis in patients suffering from sphincterotomy site bleeding from their ERCP. The rate of successful hemostasis observed in our study (87%) is on par with the reported range of successful endoscopic hemostasis rate for acute upper gastro-intestinal bleeding.
