- W2806683221 abstract "Background Healthcare lags behind other industries in identifying and learning from mistakes. National serious incident (SI) reporting and root cause analyses of adverse events offer opportunities to identify vulnerabilities in healthcare environments and gaps in training and education. This creates opportunities for training programmes to be implemented with the specific aims of addressing the system issues and learning gaps identified and to reduce the likelihood of similar incidents recurring. Context: Royal Sussex County Hospital is a major trauma centre and teaching hospital. The A&E junior doctor induction programme includes a didactic session on common mistakes / errors made in the emergency department. We reviewed the trust9s Serious Incident reports specific to the emergency department, and created simulation scenarios based on these cases. These scenarios formed the basis of a new simulation session, which has been incorporated into the A&E junior doctors teaching programme. Feedback from participating junior doctors has so far been overwhelmingly positive. In this workshop we aim to share our experience of analysing SI reports and using them to create simulation scenarios. This includes creating specific learning objectives from the SI report findings. Participants will leave the workshop with the skills necessary to create scenarios based on SI reports from their local trust and relevant to their own working environment. Learning Objectives Identifying aspects of SI reports that translate well into simulation scenarios Creating specific learning objectives from SI reports Using real life cases to create simulation scenarios relevant to a specific team and working environment Session Description 5 mins Introduction 10 mins Example scenarios based on SI reports 10 mins Small group discussion -- benefits & drawbacks of using SI reports in SBE 20 mins Small group activity -- writing learning objectives for a scenario based on an SI report 10 mins Group discussion -- challenges faced in creating simulation cases based on SI reports 5 mins. Closing Talk Educational Methods The session will draw from a number of established teaching methods including collaborative learning, learner participation and will include some didactic elements.
