Muchos factores han sido relacionados con la ocurrencia de las fallas pero estas asociaciones no han sido estudiadas. Metodos : Se realizo un estudio obser-vacional de corte en el que se incluyeron 33 individuos mayores de 18 anos con infeccion por VIH que iniciaron tratamiento antirretroviral de alta eficacia (TARVAE). Tras 24 semanas se evaluo la carga viral y los valores de CD4, asi como la adherencia al tratamiento. Otros datos (edad, genero, grado de instruccion, habito tabaquico, habito alcoholico, conducta sexual, uso de drogas y pre- sencia de enfermedad oportunista al inicio del tratamiento) tambien fueron evaluados. Resultados : La falla virologica ocurrio en 12,1% de los pacientes, mientras que 24,2% presento falla inmunologica. Bajos niveles de adherencia se relacionaron con falla inmunologica (p = 0,016). El genero, la edad, el grado de instruccion, el habito tabaquico, el habito alcoholico, la conducta sexual, el uso de drogas, las enfermedades oportunistas, la carga viral inicial, los CD4 iniciales y el tipo de TARVAE no se relacionaron con la ocurrencia de las fallas. Conclusiones : Existe una alta prevalencia de fallas terapeuticas tras el primer esquema de TAR-VAE. Bajos niveles de adherencia se asociaron significativamente con falla inmunologica. Otros factores comunmente asociados con las fallas estudiadas no se relacionan con su ocurrencia en nuestro medio. Abstract Background : In Venezuela there are few available data on the outcome of HIV infection after initiation of antiretroviral treatment and no specific documentation on the incidence of treatment failures. Many factors have been related to the occurrence of the failures but these associations have not been studied. Methods : We performed a sectional study which included 33 individuals older than 18 years with HIV who had started highly active antiretroviral therapy (HAART). After 24 weeks, they were evaluated for viral load and CD4 counts, as well as the adherence to treatment. Other data (age, gender, education level, smoking and alcohol habits, sexual behavior, drug use and presence of opportunistic disease at baseline) were also evaluated. Results : The virological failure occurred in 12.1% of patients, while 24.2% had immunologic failure. Low levels of adherence were associated with immunological failure (p = 0.016). Gender, age, educational level, smoking, alcohol habits, sexual behavior, drug use, opportunistic infections, initial viral load, CD4 and type of HAART were not associated with the occurrence of the failures. Conclusions : There is a high prevalence of treatment failures after initiation of HAART. Low levels of adherence were associated with immunological failure. Other factors commonly associated with the failures were not related to its occurrence in our setting." @default.
