• W2810733115 abstract "Background: Acute exacerbation of idiopathic pulmonary fibrosis (AE-IPF) is associated with poor prognosis in IPF; however, the nationwide status in Korean IPF patients is not well defined.Methods: Since 2016, 978 IPF patients were diagnosed from 2000 to 2017 at 17 sites were enrolled in the Korean IPF registry. Among them, clinical data were retrospectively analyzed in 844 subjects with full data set.Results: Median follow-up period was 27 months. Mean age was 66.5 years, 72.0% were male and 66.9% were ever-smokers, respectively. AE occurred in 162 (19.2%) patients, and repeatedly (≥2) in 65 (40.1% of AE patients). AE group had more female (34 vs. 26.5%, P=0.070), lower FVC (72.1 vs. 75.4% predicted, P=0.066), more decline in FVC (≥ 10% for 6 months, 24.6 vs. 13.7%, P=0.028) and shorter distance in six minute walk test (6MWT; 403 vs. 430m, P=0.049) compared to non-AE group. In multivariate logistic analysis, decline in FVC (≥ 10% for 6 months; HR, 2.171) was a only significant risk factor of AE. Median survival period was 14 months after AE, and occurrence of AE (HR, 5.519) and lower BMI (HR, 0.738) were significant prognostic factors in IPF after adjustment for FVC, DLco, decline in FVC and 6MWT distance in multivariate logistic analysis. Patients, who experienced AE repeatedly, showed higher 3-year mortality rate (18.5%) than no AE group (13.4%) or one episode AE group (5.7%, P ＜ 0.001)Conclusions: In Korean IPF patients, AE was common and showed a significant impact on mortality.
