- W2810781010 abstract "Medication-Assisted Treatment (MAT), such as Buprenorphine, is recommended for pregnant women with opioid use disorder (OUD). Without treatment, OUD can significantly impact maternal, fetal and neonatal outcomes, yet not all pregnant women are receiving MAT. Multiple delivery models complicate treatment. There is also little evidence in the literature about effective delivery strategies which consider patient-specific barriers, underscoring the significance of seeking input from patients with lived experiences. This study explored the experiences of buprenorphine use among pregnant and postpartum women with OUD to better understand critical facilitators and barriers to effective treatment. Two focus groups and semi-structured interviews were conducted over one year. Pregnant and postpartum women diagnosed with OUD (n=12), age 18 or older, were recruited from a prenatal and outpatient addictions program in Baltimore. Informed consents were obtained from University of Maryland Internal Review Board. Focus groups/interviews lasted 60-90 minutes, and discussions centered on perceptions, access, delivery and support. Data were analyzed according to an inductive grounded theory, and coded by a thematic approach, supported with NVIVO software. We heard directly from patients and identified four main themes: Access, lack of providers, adherence and support in our preliminary analysis. All women reported a scarcity of buprenorphine-waivered providers outside Baltimore City and expressed concerns about service fragmentation. Buprenorphine was instrumental in reducing cravings, drug-seeking behaviors and adherence to recommendations. Care of children replaced self-interests and women feared relapse, if taken off the medication. Educational, informational and emotional types of support were identified. Women also reported stigma as a barrier to treatment.
