- W282019985 abstract "要 旨：細胞膜microviscosityは細胞の変形能，可塑性と密接に結びついており，微小循環調節に重要な役割を果たしている。本稿では，高血圧によるchronic kidney disease(CKD)と微小循環障害との関連を，細胞膜機能異常や酸化ストレス，内皮機能不全を中心に述べる。高血圧患者の赤血球膜fluidity (microviscosityのreciprocal value)を電子スピン共鳴法にて測定してみると，腎機能低下の影響とその作用機序が明らかとなる。高血圧患者の赤血球膜fluidityは正常血圧群に比し低下(膜microviscosityが悪化)し，estimated glomerular filtration rate(eGFR)が低いほど膜fluidityの減少度は大であることがわかる。General risk factorを補正後も，eGFRは膜fluidityの独立した予測因子と考えられる。最近，酸化ストレスや内皮機能不全がCKDと密接な関連を持つことが提唱されているが，eGFRと膜fluidityの低下は，酸化ストレスの増加や血漿nitric oxide(NO)代謝産物の減少と有意に相関する。これらの結果は，腎機能低下が酸化ストレスや内皮機能不全を介して，赤血球膜のrheologic behavior異常や微小循環障害を惹起する可能性を示唆している。一方，食塩量制限や有酸素運動は，高血圧患者の赤血球膜fluidityを有意に改善する。以上の研究結果から，高血圧によるCKDは膜機能調節に重要な役割を果たし，腎硬化症に関連する血管内分泌因子の調和異常が心血管病変の成因に一部関与すると考えられる。" @default.
