The atypical parkinsonian disorders (APD) are a heterogeneous group of movement disorders characterized by prominent parkinsonism. APDs are accompanied by specific additional features such as cerebellar ataxia, early autonomic dysfunction, early dementia, pyramidal tract signs, myoclonus, supranuclear gaze palsy and apraxia, which are not features of idiopathic Parkinson9s disease (PD). Both rapid disease progression and poor or absent response to L-Dopa therapy raise the suspicion of an APD. Multiple system atrophy (MSA), progressive supranuclear palsy (PSP), corticobasal degeneration (CBD) and dementia with Lewy bodies (DLB) are the most common causes of APD. Clinical diagnosis of APD can be difficult in early stages. Although the predictive value of the widely established diagnostic criteria is high at first neurological evaluation, the sensitivity tends to be poor. At our institution, we have found that the majority of patients with APD have normal or equivocal brain MRI imaging. FDG PET/CT can provide objective evidence of the underlying brain pathophysiology that can be fit into recognizable patterns of disease. Brain PET/CT with quantification enhances diagnostic sensitivity early in the disease process allowing for potential treatment and prognostication" @default.
